New Patient Registration and dental care and treatment at the practice - For registration purposes only the following documents are required for any person of the age of 18 years or above: 1 Proof of name and address in the form of a utility bill or bank statement dated within the last 3 months. 2 Photographic ID in the form of a Passport or Picture Driving Licence. 3) Valid NHS Charge Exemption Certificate (if applicable).
